Application should be made by delivering to the surgery a medical card and/or registration form GMS1 (obtainable from the surgery) duly signed by the patient or person authorised to sign on their behalf. You will also be required to produce evidence of your identity such as driving licence or passport.

New patient Health Checks

All new patients are invited to make an appointment for a health check when they register with the Surgery.
